How to Exercise Your Rights
To exercise any of the rights described above, please contact our Privacy Officer at info@ways2thrive.com. We will respond to your request within 30 days. We will not retaliate against you for exercising your privacy rights.
Data Safeguards
We implement appropriate administrative, technical, and physical safeguards to protect your PHI against unauthorized access, use, disclosure, modification, or destruction, in compliance with the HIPAA Security Rule.
Changes to This Notice
We reserve the right to change this notice and make the revised notice effective for PHI we already hold. If we make a material change, we will post the revised notice on our website.
